Case study · ACL Rehabilitation

From ACL reconstruction to full team training.

This footballer had completed the earlier stages of rehabilitation but still needed to rebuild the strength, running capacity and confidence required for full football training.

  • 6 weeksTo progress into full team training during the reported program
  • 95%+Leg-strength symmetry recorded during rehabilitation testing
  • Full trainingProgressed into team sessions following structured preparation
  1. 01

    The starting point

The player had progressed through the initial healing and gym-based stages of ACL rehabilitation but was not yet fully prepared for team training. The remaining challenge was converting physical progress into the demands of running, changing direction and football movement.

  2. 02

    Assessment and priorities

The assessment considered lower-body strength, side-to-side differences, running capacity, movement control and confidence during football-specific tasks. These findings were used to identify the next priorities rather than progressing based on time alone.

    • Lower-body strength capacity
    • Single-leg control and confidence
    • Running and sprint tolerance
    • Change-of-direction preparation
  3. 03

    The rehabilitation plan

The program combined progressive strength training with running exposure and increasingly demanding football movement. Training was advanced gradually as the player demonstrated appropriate capacity and confidence at each stage.

    • Progressive lower-body strength work
    • Acceleration and controlled deceleration
    • Change-of-direction progressions
    • Gradual integration into team training
  4. 04

    The outcome

During the reported six-week period, the player achieved more than 95% leg-strength symmetry and progressed into full team training. The program also helped rebuild confidence during running and football-specific movement.

Key insight

Being cleared is not the same as being fully prepared.

Return-to-football decisions should consider multiple physical and football-specific factors. Strength symmetry can be useful, but it should be interpreted alongside movement quality, workload tolerance, confidence and exposure to the demands of training.

Program focus

Rebuilding the qualities football demands.

The program progressed beyond basic strength work and gradually exposed the player to the speed, movement and fatigue involved in football training.

Strength

Restoring lower-body strength and side-to-side balance.

Running

Building running capacity and sprint tolerance.

Change of direction

Progressing acceleration, deceleration and cutting.

Confidence

Rebuilding trust in the knee during football movement.
